Subject Agrees to:

Grant permission to Guest Systems Inc, hereinafter referred to as the [Company] to use my likeness, image, voice, and/or performance in any and all forms of media, including but not limited to photographs, videos, audio recordings, and digital images, for the purpose of advertising, promotion, or any other lawful purpose.

I understand and agree that Guest Systems Inc. has the right to edit, alter, copy, exhibit, publish, or distribute any media in which I appear, without compensation to me. I waive any right to inspect or approve the finished product and any accompanying written or electronic copy.

I release the Guest Systems Inc., their legal representatives, and assigns from any and all claims and liabilities arising out of or in connection with the use of my likeness, image, voice, and/or performance, including but not limited to any claims for defamation, invasion of privacy, or violation of publicity rights.

I certify that I am at least 18 years of age and have the full legal capacity to enter into this agreement.

I acknowledge that I have read and understood this release form, and I voluntarily agree to its terms and conditions.

The Company Agrees to:

Pay [Model] the following 1099 pay for Related Photos: $25.00 Per Accepted Photo Received.
